Chemical Approval request form including a substitution field

It would be good if the Chemical Approval request form included a field where the requester could point out that substitution consideration is already been evaluated

    There is now a feature that allows you to build and modify own templates for the request form.

    Customers' health care providers have been asking for this. They are interested to know whether a safer alternative has been considered if a customer wants to use e.g. a carcinogenic product.
